Library Cards

Library Cards are issued to residents of Worth. 

Requirements:
          * Photo ID
          * a current piece of mail at Worth address or 
                 a second form of ID with your Worth address.

A parent or legal guardian must be present at the time of application 
and must sign the application for children under 18 years of age.

**Patrons receive a free video or dvd rental coupon
         with a new or renewed library card**

Register Online for a Library Card

Residents of Worth are eligible to register online 
for a library card by filling out this
Registration Form. 

You will receive a temporary library card number 
that expires in 14 days. 

You must visit the library within 14 days of receiving your 
temporary library card number to complete the library card 
registration process and receive a permanent library card number.

Please bring to the library a valid photo ID and a current piece of mail 
that has your name and Worth address as proof of residence. 

Those under 18 years of age are required to come to the library 
with a parent or legal guardian in order to complete the 
library card registration. Guardians must provide legal documentation 
proving they are a minor’s legal guardian.
